Certified Respiratory Therapist - Massachusetts (Jamaica Plain)
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Schedule: 3 shifts of 12 hours each (7:00 AM - 7:30 PM), totaling 36 hours per week
Required Credentials and Compliance for Onboarding
Certifications & Licenses
- BLS (Basic Life Support): Required for onboarding. Preferred AHA certification; Red Cross accepted.
- Professional License Verification: Primary source verification required within 30 days of start. MA Conditional Licenses not accepted.
Personal Health Requirements
- N95 Fit Test: Required within 365 days of start and updated annually. Must be completed on the specific MGB form; no religious exemptions.
- MMR Vaccination or Immunity: Documentation of two measles, two mumps, and one rubella vaccine, or two MMR vaccines; or proof of immunity via blood test.
- Influenza Vaccine: Required during flu season (Sept 15 - April 30). No exemptions allowed.
- TB Test: Documentation of TB skin test or negative blood test within 3 months of start; updated if break >6 months.
- Health Statement: Physician statement signed by MD/DO/NP/PA within 365 days of start; updated if break >6 months.
- Respirator Medical Clearance (RMC) Attestation: Required if fit test is required; completed prior to fit testing.
- Hepatitis B: Strongly preferred but optional.
- Tdap Vaccine: Preferred, within last 10 years (optional).
- Positive PPD - Negative Chest X-Ray: Required if TB test positive (optional).
- TB Symptom Survey: Optional if positive PPD.
Professional Requirements
- Criminal Background Check: Within 45 days prior to start; includes international searches if applicable.
- Massachusetts CORI Check: Within 30 days prior to start; includes criminal history from age 17.
- Social Security Number Trace: Must be run with criminal check, covering last 7 years.
- National Sex Offender Registry Check: Within 30 days prior to start; updated annually.
- Exclusion Checks: Includes OIG, GSA, MassHealth, and NH Medicaid lists; updated annually.
- Education Verification: Verified through background check company; highest level of education must be confirmed.
- References: Two recent references required, at least one supervisor or above in the skillset.
- Resume: Must include past 7 years of work experience with city and state.
- Skills Checklist: Required for submission.
- Candidate Submission Coversheet: Required for submission.
- Confirmation of Assignment/POI Form: Required for all initial assignments and extensions.
This position requires compliance with all listed credentialing, health, and professional background checks prior to onboarding.
