Job Opportunity: RN Hospice - Home Hospice Care Operations
Position Details
- Employment Type: Contractor
- Contract Length: 13 weeks
- Work Schedule: Weekend program, every Saturday and Sunday from 8 AM to 8 PM
- Work Setting: Primarily remote/field work in the Milwaukee territory, visiting patients in their homes, family homes, Assisted Living Facilities, Group Homes, Nursing Homes, and occasionally hospice patients in hospitals.
Patient Population
- Hospice patients with life expectancy of 6 months or less if disease runs its normal course.
- Common diagnoses: approximately 40% cancer, 60% other medical issues.
Caseload and Visits
- RN caseload: 13-15 patients.
- Visits per day: 5-6 visits per 8-hour shift.
- Visit types: daily end-of-life visits, PRN visits for symptom management, and occasionally stable routine visits.
- This is not a case management position and does not carry a case load in the traditional sense.
Experience Requirements
- RN Experience: Minimum 2 years of RN experience required (except for inpatient hospice facility where 1 year is acceptable).
- Prior Travel Experience: Not required; first-time travelers are accepted.
Certifications Required
- Current state RN license.
- BLS certification.
Skills and Competencies
- IV skills, chest tube maintenance/pleurex drain, catheter insertion, tube feedings.
- Triage skills, wound care, wound vac management.
- Comfortable managing PIV, midline, central lines.
- Competency in end-of-life care including pain and symptom management (pharmaceutical and non-pharmaceutical).
- Experience with opioids and opioid equianalgesic dosing.
- Proficient in documentation on EMR platforms.
- Independent critical thinker and problem solver, with good judgment on when to seek assistance.
- Flexibility to work in various settings; inpatient facility provides palliative sedation.
Technology and Equipment
- EMR systems: EPIC Hyperspace/ClinDoc or Remote Client, depending on role.
- IV pumps used include PCA pump, Alaris IV pump, Curlin pump, Joey enteral pump, and Sapphire pump depending on location.
- Medication dispensing: Pyxis system at inpatient facility; hospice home program medications generally administered by family/patient/caregiver except IVs/injections managed by RN.
Orientation
- Orientation tailored to individual needs and discipline.
- May include reporting to inpatient hospice facility for equipment pick-up and training.
- Typical RN orientation includes 1 day for equipment and supplies, 2 days EPIC training, and 3-5 precepted shifts for visit nurses.
Additional Information
- Scrub color: Navy blue.
- Parking: Ground lot parking available at inpatient facility; home team members park at patient residence locations.
- Support staff such as receptionist, charge nurse, nurse educator, hospice manager, and team lead are available depending on location.
- Occasional phlebotomy may be required (peripheral stick or through a port/line).
- Respiratory therapy support available via durable medical equipment division.
- Hospice and attending physicians available for consultation.
This role is ideal for RNs with hospice experience seeking weekend field work in a supportive environment with a focus on end-of-life care.
