Licensure & Certifications

• ARRT (CT) Certification: Required
• State License: Required with primary source verification
• ACLS (AHA only): Required (especially for PACU, Post-Op, and intensive care units)
• BLS (AHA only): Required for all candidates except MLTs and non-clinical staff
• PALS (AHA only): Required as applicable (PACU, ER, CC float)
• NRP: As required
• TNCC: As required
• NIH Stroke Certification: Required for PACU & ICU

Background & Documentation Requirements

• 7-year criminal background check within 1 year of start, updated annually
• Background checks must include all locations lived/worked in past 7 years
• EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start and updated for renewals
• Transcript/Diploma or Education Verification required for all new candidates
• Proof of 2 clinical references or hospital evaluations
• Performance evaluations required if worked over 1 year; internal evaluations if over 2 years
• Complete employment application/resume
• Skills checklist updated annually for applicable units

Health & Immunization Requirements

• Initial physical exam, updated annually, attesting to ability to meet physical demands
• Tetanus/Diphtheria/Tdap vaccine within 10 years (medical or religious exemptions only)
• Negative TB test or chest X-ray (QGold or T-Spot within 6 months, updated annually)
• Measles (Rubeola), Mumps, Rubella: Positive titer required or 2 vaccines/boosters as per CDC guidelines
• Hepatitis B: Surface antibody and antigen titer within 6 months, vaccination series or declination form required
• Varicella: Positive titer required or 2 vaccines/boosters as per CDC guidelines
• Hepatitis A and C: Negative or antibody titers within 6 months
• Seasonal Flu Vaccination: Required annually (medical or religious exemptions must be approved)
• COVID-19 Seasonal Vaccine: Required; declination form required if opting out
• Drug Testing: Must be done through LabCorp within 30 days of start, updated annually

Facility-Specific Requirements

• Completion of new hire forms, confidentiality agreement, standards of behavior, IT access forms, OSHA post-test, voluntary self-identification, COVID form, hospital policies review
• Medical and religious flu exemption forms accepted
• COVID-19 and Hepatitis B declination and exemption forms required if applicable
• TB screening questionnaire required with one-year expiration

This position offers a travel contract opportunity in a large regional hospital requiring a certified Radiology/Cardiology professional with ARRT (CT) certification and multiple certifications related to life support and specialty care.
