Travel RN - Case Management

Location: Austin, Texas
Contract Length: 13 weeks
Shift: 6:30 AM - 5:00 PM
Call Requirements: Rotating weekends (No call required, potentially weekends)

Unit Information

• Unit: 3 Central – Med/Surg Unit
• Number of Beds: 34
• Number of Staff: 2 (Day Shift: 1 RN, 1 Social Worker for Case Management)
• Equipment: Computer, iPad, work phone
• EMR Systems: Meditech, Midas
• Permanent Positions Available: Yes

Job Description

• Acute hospital case management experience required
• Typical procedures:

  • Discharge planning
  • Chart review and patient screening
  • Care coordination
  • Barrier mitigation

Required Qualifications

• Registered Nurse (RN) license in Texas
• BLS certification
• Minimum 2 years acute hospital case management experience (highest required years of experience)

Preferred Qualifications

• Acute hospital case management experience of 1+ year (considered a plus but minimum 2 years required)
