Description

Job Opportunity: RN Hospice (Home-Hospice Care Operations)

Job Type

  • Contractor
  • Contract Length: 13 weeks

Job Description

  • Provide hospice nursing care in the Milwaukee territory, primarily in patients' homes and community settings such as Assisted Living Facilities, Group Homes, Nursing Homes, and occasionally hospitals.
  • Weekend program: Saturdays and Sundays, 8 AM to 8 PM.
  • Not a case management position; no caseload.
  • Visit types include daily end-of-life visits, PRN symptom management visits, and occasional stable routine visits.

Patient Population

  • Hospice patients with life expectancy of 6 months or less.
  • Common diagnoses: approximately 40% cancer, 60% other medical issues.

Patient Ratios

  • Field/Remote RN caseload: 13-15 patients.
  • Visits per day: 5-6 visits per 8-hour shift.
  • Inpatient hospice RN ratio: 1 nurse to 6-9 patients depending on shift.

Required Qualifications

  • Registered Nurse (RN) license in the state of practice.
  • BLS certification.
  • Minimum 2 years of RN experience (except for inpatient hospice facility which may accept 1 year).
  • First-time travelers are accepted.
  • Prior travel experience is not required.

Skills and Competencies

  • IV skills, chest tube maintenance/pleurex drain care, catheter insertion, tube feedings.
  • Triage skills and wound care including wound vac.
  • Comfortable managing PIV, midline, central lines.
  • Competency in end-of-life care including pain and symptom management (pharmaceutical and non-pharmaceutical).
  • Experience with opioids and opioid equianalgesic dosing.
  • Proficient in documentation using EMR platforms.
  • Independent critical thinker and problem solver with good judgment on when to seek assistance.
  • Flexibility to work in both field and inpatient settings.
  • Experience with palliative sedation in inpatient hospice.

Technology and Equipment

  • EMR systems: EPIC Hyperspace/ClinDoc or Remote Client depending on role.
  • IV pumps: PCA pump, Alaris IV pump, Curlin pump, Joey enteral pump, Sapphire pump depending on location.
  • Medication dispensing: Pyxis system in inpatient facility; hospice home program patients/families generally administer medications except IVs/injections managed by RN.

Orientation

  • Individualized based on discipline and needs.
  • May require reporting to inpatient hospice facility for orientation.
  • Typical RN orientation includes equipment pick-up, online training, 2 days EPIC training, and 3-5 precepted shifts for visit nurses.

Additional Information

  • Scrub color: Navy blue.
  • Parking: Ground lot parking at inpatient facility; home team members park at patient residence locations.
  • Support available includes receptionist, charge nurse, nurse educator, hospice manager, team lead, preceptor, and hospice physician.
  • Phlebotomy may be infrequently required.
  • Respiratory therapy support available via DME division.
