Job Opportunity: Hospice RN Case Manager
Employment Type: Contractor
Contract Length: 13 weeks
Job Description
- Provide nursing case management for home hospice patients, including those in home settings, assisted living facilities, skilled nursing facilities, group homes, independent living, and hospitals for General Inpatient Care (GIP).
- Manage caseloads averaging 14-16 patients per RN.
- Patient population primarily includes end-stage patients.
Work Schedule
- Shift: 1st shift, 8:00 AM to approximately 4:30 PM, Monday through Friday.
- No weekend rotation.
- No on-call duties.
- No holiday work expected.
Required Qualifications
- Valid RN license in Wisconsin.
- Minimum 2 years of Hospice Palliative Care RN experience, including experience within the last year.
- Basic Life Support (BLS) certification.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
- Valid driver’s license and auto insurance.
Skills & Experience
- Experience with home hospice care preferred; first-time travelers may be considered but preference given to experienced candidates.
- Ability to work independently within the community, traveling to patient homes.
Orientation & Support
- One week of unit orientation provided.
- Team-oriented culture with emphasis on teamwork and support.
Technology & Equipment
- Electronic Medical Records (EMR): Epic and Rover.
- Use of IV pumps and medication dispensing equipment.
- Monitoring devices including chair and bed alarms.
- Home health supplies carried in bags and trunks.

- Scrub color: Blue or black.
- Free parking available at office locations.
This position is ideal for an experienced RN with a background in home hospice care seeking a community-based case management role with a stable weekday schedule.
