Requirements

• Valid RN license in New York
• Minimum of 1 year of Emergency Department (ED) experience within the last year
• Associate of Science in Nursing (ASN) / Associate Degree in Nursing (ADN)

Certifications Required

• ACLS - Advanced Cardiac Life Support (AHA only)

• State License Verification
• Color Blind Testing (within 1 year of contract start date)

Health Documents Required

• Current Health Form/Physical Fitness for Duty (signed and dated by provider within a year of start)
• N95 Fit Mask (must be updated annually)
• Negative TB or Chest X-Ray (PPD or IGRA test within 12 months of start)
• Vaccination documentation for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
  • Flu Seasonal Vaccination (documentation required by 12/3)
• Snellen Eye Exam (within 1 year of contract start date)
• Drug Testing (within 30 days of start date)

Background and History Checks

• Criminal Background Check (no older than 3 months)
• Employment References (minimum of 2)
• Current Resume/CV
• Education Verification (if applicable)
• State Specific Searches (required within 3 months of start)

Additional Information

• Fingerprinting may be required for Mental Health and Long-Term Care units.
• Candidates must verify at least two past employments.
• Academic verification from a third party is required as per position.
