Description
Job Opportunity: Home-Hospice Care Nurse (Contract)
Position Details
- Employment Type: Contractor
- Contract Length: 13 weeks
- Location: Remote field work in community settings including homes, Assisted Living Facilities, Group Homes, and Nursing Homes within Ozaukee and Washington counties.
- Shift: Monday to Friday, 8:00 AM – 5:00 PM (must be available by phone during these hours)
- Daily huddle at 7:50 AM (can attend by phone or Teams)
- Flexible schedule options available (M-F or Tues-Sat)
- Weekend rotation: No weekends required, but open to working Saturdays if needed
- No call or holiday work required
- Orientation: Up to 2 weeks, based on comfort level
Patient Population & Caseload
- Patients primarily in community settings with high acuity needs such as:
- Pleurx drains, PCA pumps, IV antibiotics, TPN, Foley catheters, wounds, wound VAC, chest tubes, various drains
- RN Case Manager caseload: 22-25 patients per week; approximately 6 visits per day
- Must communicate with leadership if unable to keep up with caseload
Required Qualifications
- License: Registered Nurse (RN)
- Certifications: Basic Life Support (BLS)
- Experience: Minimum 1 year of Home Health experience required
- First-time travelers accepted if they meet the 1-year Home Health experience requirement
- Skills:
- Patient and family education
- Provider communication
- Disease management
- Knowledge of troubleshooting pleurx drains, nephrostomy tubes, tracheostomies, PCA pumps, gastric suctioning (will not perform these tasks but must understand troubleshooting; training provided onsite)
- Technology: Experience with EPIC EMR preferred
- Other Requirements: Must be N95 fit tested prior to arrival
