Job Opportunity: Physical Therapist - Acute Care

Location: Cottonwood, Arizona
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Description

• Provide physical therapy services in an acute care setting.
• Work schedule: Four 10-hour shifts per week (Saturday, Sunday, Monday, Tuesday).
• Average daily census: 11 patients.
• Weekend work required (at least one weekend day).
• May be asked to float to surrounding clinics.
• Blocked scheduling with guaranteed 40 hours per week.
• Facility may call off up to 6 times during the 13-week assignment.
• Business casual attire.
• Charting system: Cerner.
• Free parking available.

Required Qualifications

• Valid Physical Therapist license in Arizona.
• Minimum 1 year of Physical Therapist experience within the last year.
• BLS certification issued by the American Heart Association.

Preferred Skills

• Generalist PT comfortable in acute care.
• Flexibility to pivot between patient needs and evaluations.

Facility Highlights

• Short Term Acute Care facility.
• Teaching hospital with Level IV trauma designation.

Compliance & Documentation Requirements

• Employment application with 3 years of work history; gaps over 30 days must be documented.
• Two professional references required (one from most recent employer within 1 year).
• Background checks including 7-year history, SSN trace, criminal search, sex offender search.
• Various certifications and competency exams must be current and valid.
• Physical exam within 1 year confirming ability to perform essential job functions.
• Fit mask test (N95) required within 1 year and updated annually.
• Immunizations and titers required or declination forms accepted for:

  • Tetanus/Diphtheria/Tdap
  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
  • Influenza (2025-26 season)
  • COVID-19 vaccination or declination.
• Drug testing (10-panel) within 30 days of start and updated annually.
• Facility-specific forms and policies must be reviewed and signed prior to start, including confidentiality, substance abuse screening, professional attire, and security questions.

Additional Notes

• Locals are accepted.
• Return-to-Office (RTO) restrictions require unit manager approval.
• No prior travel experience required.
