Description

Travel Registered Nurse (RN) - Case Management

Location: Lone Tree, Colorado
Employment Type: Travel
Contract Length: 13 weeks


Unit Information

  • Unit: Case Management
  • Shift: Day shift, 8:00 AM – 4:30 PM (may include weekend coverage)
  • Patient Ratios: 18-25 patients per case manager
  • Staffing: 2-3 case managers on the floor
  • Typical Team: 1 charge nurse, 3 RNs, 2 CNAs, 1-3 sitters
  • Floor: 5th floor, Main Hospital
  • EMR Systems: Meditech, CarePort

Job Description

  • Perform patient rounding focusing on high-risk discharge planning needs
  • Collaborate with physicians and nurses to develop safe discharge plans
  • Coordinate with skilled nursing facilities, home health, hospice, and LTACH providers
  • Arrange medication placement and transportation for patients
  • Conduct patient assessments and participate in patient-family conferences

Required Qualifications

  • Valid RN license in Colorado
  • Minimum 2 years of case management experience, including experience within the last year
  • BLS certification
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
  • Experience in discharge planning
  • Experience working in home health, long term acute care (LTACH), skilled nursing facilities (SNFs), and hospice settings
