Home-Hospice Care Liaison (Contract)

Job Overview

• Employment Type: Contractor
• Contract Length: 13 weeks
• Patient Types: Adults with heart failure, cancer, and other end-of-life disease processes
• Work Setting: Private homes, skilled nursing facilities (SNF), assisted living for home hospice admissions
• Average Patient Load: 2-3 patients per shift

Required Qualifications

• Valid RN license in Wisconsin
• Minimum 2 years of hospice experience, including experience within the last year
• Associate of Science in Nursing / Associate Degree in Nursing
• BLS certification
• Experience with end-of-life symptom management, including pain and shortness of breath
• Comfortable with end-of-life medication management
• Experience working with Epic EMR (Hyperspace and Remote Client) for charting and secure communication
• Valid Driver’s License, auto insurance, and reliable vehicle

Orientation & Training

• New Employee Orientation (NEO) provided
• 40 hours of unit orientation with staff member

Schedule & Work Conditions

• Monday to Friday shifts, 8 or 12 hours (08:00–16:30 or 08:00–20:00)
• No weekend rotation, on-call, or holiday expectations
• Rotating days off if working 12-hour shifts
• Travel between patient locations typically within 45 minutes

Work Environment & Support

• Supervisor/Manager available by phone and chat
• Liaison team and Hospice Medical Directors available for support
• Dress code: Business casual or professional scrubs
• Parking varies depending on client home location

This position is suitable for healthcare professionals with hospice experience who are comfortable working independently in community settings and managing end-of-life care coordination.
