Description

Job Opportunity: Home-Hospice Care Liaison

Employment Type: Contractor
Contract Length: 13 weeks


Job Overview

  • Serve as a hospice liaison, monitoring referral queues and consulting with patients referred from hospitals or skilled nursing facilities (SNFs) for hospice care.
  • Provide assessments and recommendations on appropriate hospice settings for patients transitioning to hospice in hospital settings.
  • May admit patients to hospice services within the hospital.
  • Assigned to 1-2 hospitals depending on size and patient census.
  • Typical patient load: 2-3 patients per day.

Patient Population

  • Adults with diagnoses such as heart failure, cancer, and other end-of-life disease processes.

Required Qualifications

  • Valid RN license in Wisconsin
  • Minimum 2 years of Hospice Palliative Care RN experience, including experience within the last year
  • Associate of Science in Nursing / Associate Degree in Nursing
  • BLS (Basic Life Support) certification
  • Experience with Epic EMR systems
  • Comfortable educating providers on how to manage end-of-life medications
  • Understanding of different levels of hospice care (general inpatient, routine, respite)

Skills and Competencies

  • Symptom management for end-of-life conditions (e.g., pain, shortness of breath).
  • Comfortable with end-of-life medications and educating providers on management.
  • Ability to collaborate and communicate effectively with physicians.
  • Knowledge of hospice benefits, philosophy, and different levels of hospice care (general inpatient, routine, respite).
  • Understanding of various hospice service levels.

Work Environment & Support

  • Travel to 1-2 hospitals daily; average driving time approximately 45 minutes one way.
  • Provided with laptop and phone.
  • Support available from supervisors/managers via phone and chat.
  • Liaison team and Hospice Medical Directors available for consultation.

Orientation & Scheduling

  • 40 hours of unit orientation with staff member.
  • No weekend rotation, on-call, or holiday work required.
  • Work hours: Monday to Friday, 8:00 AM – 6:00 PM.
  • Rotating day off each week; occasional block scheduling may be possible but not guaranteed.

Additional Information

  • Dress code: Business casual or professional scrubs; white coat preferred but not required.
  • Parking is free.
