Job Opportunity: Primary RN Case Manager - Hospice
Location: Grand Junction, Colorado
Employment Type: Contract
Contract Length: 13 weeks
Job Description
As a Primary RN Case Manager in hospice care, you will provide compassionate, patient-centered care that supports patients and their families during challenging times. This role involves:
- Delivering expert symptom management and holistic care across a variety of settings including private homes, nursing homes, group homes, and assisted living facilities.
- Acting as a generalist in all diseases with a specialty in symptom management.
- Leading an interdisciplinary team to coordinate care for an assigned caseload of patients.
- Teaching and supporting patients and caregivers to identify personal goals of care and build confidence in caregiving.
- Working primarily Monday through Friday, 8am-5pm, with occasional weekend backup and on-call duties.
This position offers the opportunity to connect deeply with the reasons you became a nurse and make a meaningful impact in hospice care.
Required Qualifications
- Valid Registered Nurse (RN) license in Colorado
- Minimum of 2 years of hospice nursing experience, including experience within the last year
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)

- Prior travel experience is not required for this position.
- The role emphasizes compassionate care, teaching, and leadership within an interdisciplinary team.
- Schedule is primarily weekday with some flexibility for weekend coverage.
