Job Opportunity: Case Manager (Registered Nurse)

Location: Harvey, Illinois

Contract Length: 6 weeks

Job Description

• Conduct initial patient and family assessments on every inpatient to determine psycho-social needs and discharge plans.
• Complete Care Coordination Assessment within the first 48 hours of admission to identify need for Care Coordination intervention.
• Document significant patient data, problems, assessment goals, treatment plans, actions taken, and follow-up activities weekly or more frequently if needed.
• Participate as a member of Multidisciplinary Rounds.
• Provide consultative support to staff regarding patients and families.
• Communicate clinical status updates to patients, families, and involved individuals.
• Regularly evaluate treatment plan appropriateness and progress toward goals; communicate updates with patient and/or family.
• Review final discharge plans and instructions with patient, family, or responsible party, documenting in the medical record.
• Maintain records on each patient’s discharge date and any referred services.
• Coordinate discharge planning with the rehabilitation team.

Required Qualifications

• Valid RN license in Illinois
• Minimum 1 year of Case Management experience, including experience within the last year
• BLS (Basic Life Support) certification issued by the American Heart Association
• State License 1 Verification
• NURSYS Verification (RN, LPN/LVN, AP)
• Knowledge and experience with NIHSS (National Institutes of Health Stroke Scale)
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Experience with discharge planning processes and coordination
