Job Description

This position is for a Registered Nurse working as a Case Manager in a Skilled Nursing Facility setting within a hospital. The unit focuses on short-term rehab patients (14-21 days) requiring nursing and medical support, including physical, occupational, and speech therapy as needed.

Key Responsibilities

  • Completing Minimum Data Set (MDS) assessments (current experience within last 3 years required; must be hands-on, not management)
  • Baseline care plan completion and presentation to residents
  • Care plan meetings and development
  • Submission and validation of assessments
  • Oversight of certification/recertification process
  • Proper diagnosis coding for new admissions
  • Education on MDS changes
  • Triple check/utilization review ensuring accurate billing documentation
  • Admission referral review and insurance preauthorization support
  • Coordination of care and discharge planning
  • Clinical documentation improvement and medical necessity determination
  • Appeals, denials, audits, benefits eligibility, prior authorizations, and quality improvement reviews

Patient Population

  • Patients with diagnoses ranging from generalized weakness to post-surgical aftercare, including surgical wounds and pressure ulcers.

Work Environment

  • Ratio: 1:12
  • Average daily census: 9 patients
  • Rooms/Beds: 12
  • Shift: 8-hour days, 08:30 - 17:00
  • Call, float, and weekend requirements: None

Unit Systems and Equipment

  • Pyxis medication system
  • EPIC electronic health record system (including chat/Voalte and Rover)
  • No use of restraints on the unit

Orientation

  • 24 hours on the unit after one week of hospital orientation

Dress Code

  • Navy blue scrubs (not provided)

Required Qualifications

  • Valid Registered Nurse (RN) license in North Carolina
  • Bachelor of Science in Nursing (BSN)
  • Minimum 3 years of case management experience, including case management experience within the last year
  • Minimum 3 years of hands-on Minimum Data Set (MDS) completion experience within the last 3 years (not a management role)
  • Basic Life Support (BLS) certification issued by American Heart Association or American Red Cross
  • Pediatric Advanced Life Support (PALS) certification issued by American Heart Association or American Red Cross
  • Orientation Safety Manual Acknowledgement
  • Experience in Long Term Acute Care (LTAC) settings
  • Experience in Rehabilitation settings
  • Experience in Skilled Nursing and Managed Care settings
  • MDS coordinator experience
  • Experience with admission criteria, appeals and denials, audits, and benefits eligibility
  • Care coordination experience
  • Clinical documentation improvement experience
  • Ability to determine medical necessity per evidence-based guidelines
  • Needs assessment experience including home health, hospice, and skilled nursing

Preferred Qualifications

  • Experience with Diagnosis Related Groups (DRG), HIPAA guidelines, NCQA, OBRA, and Risk Adjustment Processing Systems (RAPS)
  • Experience educating nursing staff on duties
  • Additional certifications such as RAC-CT and ICD-10 coding

Traveler Experience

  • First-time travelers are accepted; prior travel experience is not required

Additional Information

  • No call, float, or weekend requirements
  • 40 hours per week can be offered
  • Must accept a 13-week contract length
  • No Return to Origin (RTO) allowed after booking; call-offs or additional RTO may result in cancellation

Compliance and Documentation Requirements

Candidates must complete and provide documentation for the following (among others):

  • Background checks within required timeframes
  • Drug testing within 6 months of start
  • Immunizations and titers (including TB, Rubeola, Mumps, Rubella, Hepatitis B, Varicella)
  • Flu vaccination or approved exemption during flu season
  • Physical exam (as per travel agency requirements)
  • Competency exams including medication and EKG/dysrhythmia testing
  • Various facility-specific orientation and compliance trainings
  • Photo for badge creation (passport style, white background, business attire)
  • Proof of completion of required certifications and education modules
