Description

Job Opportunity: RN Hospice Supportive Care Team

Employment Type: Contractor
Contract Length: 13 weeks


Job Description

  • Provide in-home hospice care visits including PRN, routine, and death visits.
  • No on-call duties and no weekend work, except for Labor Day coverage.
  • Patient visits occur within a defined service area (multiple nearby communities).
  • Attend after-hour visits called into triage line for symptom management, falls, or death visits; some issues may be resolved via phone without in-person visit.
  • May occasionally cover slightly outside assigned territory if closer to a call, but never more than an hour away.

Unit Information

  • Minimum 2 years of experience required in Home Health or Home Hospice.
  • First-time travelers accepted if they have case management experience in the home setting.
  • Patient types include failure to thrive, advanced cancers, end stage COPD, terminal illness, and other end-stage disease processes.
  • Patient ratio for RN typically 0-6 depending on call-ins.

Required Qualifications

  • Active Registered Nurse (RN) license in the state of Wisconsin.
  • Basic Life Support (BLS) certification.
  • Valid driver’s license, own vehicle, and auto insurance.
  • Comfortable performing focused assessments, critical thinking, and troubleshooting (e.g., Foley catheter care, feeding tubes, wound care, post-fall assessments).
  • Experience with death visits and communication with medical examiners or coroners.
  • Ability to administer narcotic pain medications and identify potential drug diversions in the home.

Orientation & Training

  • New Employee Orientation (NEO) at territory office.
  • One week (3 shifts) orientation with a Supportive Care Team nurse.

Scheduling

  • Shift times: 8:00 PM to 8:00 AM, 3 shifts per week.
  • Weekend rotation every other weekend (Saturday/Sunday night).
  • Holiday coverage: 1 summer and 1 winter holiday expected.
  • Two-week block schedule cycle.

Support & Resources

  • 24/7 access to Lead RN for support.
  • Other Supportive Care Team members available by phone.
  • Provided equipment includes badge, laptop, cell phone, and trunk kit.
  • Electronic Medical Record system: Epic (Rover).
  • IV pump (Sapphire) rarely used in home setting.

  • Scrub color: Navy blue.

Note: Prior travel experience is not explicitly required. First-time travelers with appropriate home care case management experience are welcome.
