Job Opportunity: Registered Nurse - Home Health

Location: Hawthorne, New York
Employment Type: Travel/Contract
Contract Length: 13 weeks (approximately 3 months)

Job Description

  • Provide and coordinate home nursing care according to physician orders and agency policies.
  • Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
  • Conduct home visits with an average patient load of 5-8 patients per day.
  • Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
  • Must have reliable transportation, a current driver's license, and required liability insurance.
  • Travel radius up to 75 miles from the home clinic; approximately 150-200 miles traveled weekly.
  • Charting and documentation using HomeCare HomeBase (HCHB) and OASIS (for RNs).
  • Typical schedule: Monday to Friday, 8 AM to 5 PM; some positions may have weekend rotations every 4-6 weeks and on-call requirements.
  • Dress code: Scrubs.

Required Qualifications

  • Minimum 1 full year of recent Adult Home Health experience or 1 full year Hospice experience (depending on specialty).
  • Recent specialty experience required; first-time travelers are not accepted.
  • Current Registered Nurse (RN) license in hand; pending licenses are not accepted.
  • CPR and BLS certification required.
  • Proficiency with HomeCare HomeBase (HCHB) within the last 6 months; if not current, retraining is required.
  • RNs must be proficient in OASIS.
  • Must have a valid driver's license and personal or auto insurance policy under the candidate's name or list the candidate as a driver.

Additional Requirements

  • Must complete various background checks, including but not limited to:
    • Standard 7-year background check.
    • Fingerprinting (state-specific requirements apply).
    • Drug screening (10-panel).
    • Education verification.
    • Sex offender and fraud/abuse screenings.
  • Health documents required prior to start:
    • Physical exam within 12 months.
    • Negative TB test or chest X-ray within 12 months.
    • Proof of immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella.
    • Flu vaccination or documented declination.
    • COVID-19 vaccination or documented declination.
  • Must review and sign various compliance, confidentiality, and policy documents prior to start.
  • Two professional references required.
  • Work history verification for the last 7 years or at least two most recent employers.

Work Environment & Expectations

  • Home health nursing with direct patient care in patients' homes.
  • Must be comfortable working independently and managing a patient caseload.
  • Ability to travel within the assigned territory.
  • Some positions may require weekend work and on-call availability; specifics to be discussed during interview.

  • No guaranteed hours; if patient census is low, floating to nearby clinics may be arranged.
  • Candidates must be prepared to complete all required documentation and compliance training prior to assignment start.
  • Contract length is typically 13 weeks.
