Description
Home-Hospice Care Liaison
Employment Type: Contractor
Contract Length: 13 weeks
Job Description
- Conduct admissions and visits for a home hospice program, primarily in private homes, skilled nursing facilities (SNF), and assisted living.
- Average patient census: 2-3 patients per shift.
- Responsible for completing two admissions per shift or conducting visits if admissions are unavailable.
- Provide education and symptom management related to end-of-life care, including pain and shortness of breath.
- Work closely with physicians for orders and communicate with liaisons and case managers.
- Comfortable managing end-of-life medications and educating providers on their use.
- Territory primarily covers Sheboygan/Manitowoc area, with occasional travel to other nearby locations.
- Floating may be required to areas such as Burlington and Walworth, with travel times typically under 45 minutes between homes/facilities.
Patient Population
- Adults with diagnoses such as heart failure, cancer, and other end-of-life disease processes.
Schedule
- Monday through Friday.
- Shift options: 8-hour (8:00 AM – 4:30 PM) or 12-hour (8:00 AM – 8:00 PM) shifts.
- No weekend, holiday, or on-call requirements.
- Rotating days off if working 12-hour shifts.
Orientation & Support
- New Employee Orientation (NEO).
- 40 hours of unit orientation with staff.
- Supervisor/manager available by phone and chat.
- Liaison team and hospice medical directors available for support.
Technology
- Electronic Medical Record (EMR): EPIC (Hyperspace and Remote Client) required for charting and secure communication.
Required Qualifications
- Valid RN license in Wisconsin.
- Minimum 2 years of hospice palliative care RN experience, including experience within the last year.
- Associate of Science in Nursing / Associate Degree in Nursing.
- BLS certification.
- Experience with Epic.
