Outpatient Physical Therapist - Travel/Contract
Location: Cottonwood, Arizona
Contract Length: 13 weeks
Job Details
- Specialty: Therapy/Rehabilitation
- Shift: Monday to Friday, either 5x8-hour shifts or 4x10-hour shifts (typically 4x10, 8:00 AM - 6:30 PM)
- Hours per Week: 40 hours guaranteed
- Daily Patient Load: Approximately 11 patients
- Weekend Requirement: As needed
- On Call: No call required
- Floating: May be asked to float to surrounding outpatient clinics
- Blocked Scheduling: No
- Locals Accepted: No
- RTO Restrictions: Unit Manager’s approval required
- Work Environment: Hospital-based ambulatory clinic supporting Orthopedics, Geriatrics, Cardiopulmonary, Neurological, Lymphedema, and Pelvic Health
Required Qualifications
- Valid Physical Therapist license in Arizona
- Minimum 1 year of experience as a Physical Therapist, with experience within the last year
- Basic Life Support (BLS) certification issued by the American Heart Association
Compliance and Documentation Requirements
- Background Checks:
- 7-year background check within 30 days of start date including SSN trace, counties, states, nationwide criminal search, and national sex offender search
- EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start
- Health Requirements:
- Physical exam within 1 year confirming ability to perform essential job functions
- Fit mask test (N95) within 1 year and updated annually; onsite fit testing available if needed
- Negative TB test (PPD, Q Gold, or Tspot) within 1 year and updated annually; chest x-ray required for past positives
- Immunizations or proof of immunity required for:
- Tetanus/Diphtheria/Tdap (within last 10 years or declination)
- Measles (Rubeola), Mumps, Rubella (positive titer or 2 MMR vaccines)
- Hepatitis B (positive titer, 3-dose vaccine series, or declination)
- Varicella (positive titer or 2 vaccines)
- Seasonal flu vaccination or signed declination form required for 2025-2026 season; decliners must wear a mask when not eating or drinking
- COVID vaccination series or declination with facility-specific form
- Drug Testing: 10-panel drug screen within 30 days of start and updated annually
- Competency Exams: Must be completed within 1 year of start with minimum passing scores (medication test, core parts I-III, competency exam)
- Color Blind Testing: Required for all candidates starting 1/19/26 and after, with 100% passing score via online testing
Additional Facility Requirements
- Photo for badge (clear headshot with preferred clear background)
- Completion and signing of facility-specific job description form prior to start
- Review and sign policies including:
- Workforce member confidentiality agreement
- Substance abuse screening policy
- Professional attire and appearance policy
- Security question form (at least one week prior to start)
- Travel information verification form (prior to start)
Work Environment and Culture
- Facility is a short-term acute care hospital with 87 staffed beds
- Teaching hospital with Level IV trauma designation
- Business casual dress code
- Electronic charting system: Cerner
- Free parking available
- Adherence to quality and patient safety standards including medication administration (BCMA) expected
This position is suitable for first-time travel therapists. Prior travel experience is not required.
