Location: Cottonwood, Arizona

Job Details

• Specialty: Therapy/Rehabilitation (Physical Therapy - Acute Care)
• Shift: 4 days per week, 10-hour shifts (Wednesday to Saturday, 8:00 AM - 6:00 PM)
• Average daily patient census: 11
• Guaranteed 40 hours per week
• Weekend requirement: Must work at least one weekend day
• Floating: May be asked to float to surrounding clinics
• Blocked scheduling: Yes
• Locals accepted
• Time off requests require unit manager approval
• Facility may call off up to 6 times during the 13-week assignment
• On-call: Not required

Required Qualifications

• Valid Physical Therapist license in Arizona
• Minimum 1 year of Physical Therapist experience within the last year
• BLS certification issued by the American Heart Association
• Experience with co-treatments

Required Documentation & Compliance

• Employment application covering last 3 years with documented gaps over 30 days
• Two professional references (one from most recent employer within 1 year, second within 2 years)
• Background checks including 7-year history, SSN trace, criminal, sex offender, EPLS/GSA/SAM, and HHS/OIG within 30 days of start
• AZ fingerprint card required prior to start
• Physical exam within 1 year confirming ability to perform essential job functions
• Drug test (10-panel) within 30 days of start and updated annually
• Immunizations and titers required or declinations accepted with facility-specific forms:

  • Tetanus/Diphtheria/Tdap (preferred within 10 years)
  • Measles (Rubeola), Mumps, Rubella (proof of immunity or 2 MMR vaccines)
  • Hepatitis B (titer, vaccine series, or declination)
  • Varicella (proof of immunity or 2 vaccines)
  • Seasonal flu vaccine or signed declination form (mask required if declined)
  • COVID vaccine series or declination with facility-specific form
• Negative TB test (PPD, Q Gold, or T-Spot) within 1 year and updated annually; chest x-ray and TB questionnaire if previously positive
• Fit mask (N95) within 1 year and updated annually; PAPR not accepted except in rare department-approved cases
• Completion of facility-specific job description form prior to start
• Review and sign facility policies including confidentiality, substance abuse screening, professional attire, and security questions
• Photo for badge required (clear headshot preferred)

• Department operates 7 days a week; weekend work is mandatory
• Candidate should be a generalist comfortable with acute care demands
• No prior travel experience required; first-time travelers accepted
