Medical Surgical LPN / LVN Travel/Contract Position
Location: Rochester, New York
Employment Type: Travel/Contract
Shift: Night (7:00 PM – 7:00 AM), 4 shifts per week, 12 hours per shift (48 hours/week)
Contract Length: 13 weeks
Job Summary
Provide nursing care for medical-surgical patients including those with COPD exacerbation, infections, failure to thrive, and social issues. Must be able to manage own patient assignments with RN support in an acute care setting.
Required Qualifications
- Valid LPN / LVN license in New York (NY is not a Compact state; pending licenses not accepted)
- Minimum 1 year of Long Term Care experience within the last year
- Medical-Surgical nursing experience
- Basic Life Support (BLS) certification issued by the American Heart Association
- Experience with EPIC electronic health record system
Scheduling and Work Requirements
- Night shift: 7:00 PM to 7:00 AM
- Weekend requirement: Every other weekend
- Holiday requirement: Available to work every other holiday if needed
- Floating: May be required to float within skillset
- Call-off policy: Up to three shifts may be canceled by client during the 13-week contract without charge
Unit Details
- Patient ratio: 1:5-6 during day shifts
- Patient population: Medical patients with COPD exacerbation, infections, failure to thrive, and social issues
- Dress code: Scrubs in any combination of navy blue and white
Health and Compliance Requirements
- Government-issued photo ID required
- 7-year criminal background check (including felony, SSN trace, national criminal, sex offender, education verification) within 3 months of start
- EPLS/GSA/SAM and HHS/OIG checks required within 3 months of start and run monthly
- Foreign education equivalency evaluation if applicable
- Physical exam within 1 year of start, updated annually
- Fit mask test within 1 year of start, updated annually (specific mask types accepted)
- Negative TB test or chest X-ray within 1 year of start; TB questionnaire accepted in lieu of annual renewal
- Immunizations or titers required for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B (surface antibody positive titer, completed series, or declination)
- Varicella
- Drug screen within 30 days of start
- Seasonal flu vaccination or signed declination required during flu season
- Snellen eye exam within 1 year of start
- COVID vaccination copy preferred but not required
Documentation and Verification
- Education verification from primary source required (hard copies not accepted)
- State license primary source verification within 30 days of start
- NURSYS verification within 30 days of start
- Medication/pharmacology competency exam as required
- Color blindness testing required once
- Competency exams as required by specialty
- Orientation safety manual acknowledgement required
- Photo for badge required (passport-style with solid background)
- Employment application and work history verification required
- References required for travel agency candidates (2 references within 3 years)
Additional Notes
- First-time travelers accepted
- Locals accepted with adjusted pay rate
- Fingerprinting and additional background checks may be required for certain units (mental health, behavioral health, home health aide, long-term care)
- Candidate must complete all health-related requirements and lab work within 5 days of contract signing
