Licensed Practical Nurse (LPN) - Contract Position

Job Summary

The Licensed Practical Nurse (LPN) performs and documents assessments appropriate to licensure, ensures completion of health history and admission processes, provides patient care including personal cares, and identifies discharge planning and education needs. The LPN communicates patient progress and adjusts workload accordingly.

Key Responsibilities

• Assist registered nurses or other licensed practitioners in identifying patient needs, care priorities, and goals considering patient demographics and beliefs.
• Contribute to evaluating and modifying the plan of care by observing and communicating patient responses.
• Demonstrate competency in department-specific functions following standards of care, policies, and procedures.
• Function as a member of the multidisciplinary team to implement integrated patient-centered care plans.
• Delegate nursing interventions to qualified personnel safely.
• Identify early warning signs of patient condition changes and report appropriately.
• Implement nursing interventions and prescribed medical regimens timely and document care accurately.
• Collaborate and communicate relevant patient information with patients, families, and team members to ensure quality and continuity of care.
• Report patient pain symptoms promptly according to procedures.
• Participate in health education approved by licensed practitioners.
• Assist in discharge planning, education, and counseling to optimize patient and family understanding.

Required Qualifications

• Valid LPN / LVN license in Minnesota
• Minimum of 2 years of clinic nursing experience
• Clinic experience within the last year

• This position requires working 36 hours per week on the day shift.
• Contract length is approximately 13 weeks.
