• Weekly Estimated Pay: $1,570 - $1,650
• Location: Roseau, MN (56751) — travel LPN job in Minnesota
• Assignment Duration: 13 weeks (May 18, 2026 – August 17, 2026)
• Hours per Week: 36 guaranteed hours
• Shift: Day shift (12-hour shifts)

Job Requirements

• Active Licensed Practical Nurse (LPN) license in Minnesota
• Minimum 1 year of LPN experience in a clinical or outpatient clinic setting
• Certification in Nursing or equivalent credential as required
• Ability to work collaboratively within a multidisciplinary healthcare team
• Strong clinical communication, patient education, and documentation skills

Responsibilities

• Perform and document patient assessments appropriate to LPN licensure in an outpatient clinic or primary care setting
• Complete health history and admission processes; ensure timely interventions or referrals as needed
• Identify discharge planning and education needs; provide patient care and personal care services
• Assist Registered Nurses and other practitioners in identifying patient needs and care priorities
• Gather, observe, record, and communicate patient responses to nursing interventions
• Modify and update care plans in collaboration with licensed practitioners
• Demonstrate competency in department-specific functions according to policies, standards, and procedures
• Assign or delegate nursing interventions when appropriate and within scope of practice
• Recognize early warning signs of changes in patient condition and report to the care team promptly
• Implement nursing interventions and prescribed medical regimens safely and promptly
• Communicate relevant patient information with patients, families, and team members to support continuity of care
• Participate in health education and discharge planning to optimize patient and family understanding
