Description

Registered Nurse Case Manager - Hospice

Job Type

  • Contractor
  • Contract length: 13 weeks
  • Shift: 8-hour day shift (8:00 AM – 4:30 PM), Monday through Friday
  • Hours per week: 40
  • Weekend backup rotation: approximately 1 out of every 5 weekends
  • On-call schedule: 1 night per week plus alternating weekends (same as backup)
  • No holiday work expected
  • Time off requests approved by RTO team with manager notification
  • Floating: No
  • Orientation: 1 week unit orientation (NEO)

Job Description

  • Provide home health services to patients in hospice specialty areas
  • Manage caseloads and conduct initial patient assessments (physical, social, environmental)
  • Establish and update plans of care based on physician orders and patient needs
  • Deliver direct patient care, adjust nursing care plans, and evaluate outcomes
  • Communicate regularly with physicians and healthcare team members regarding patient status and care plans
  • Facilitate discharge planning and ensure continuity of care post-discharge
  • Complete documentation including visit assessments, OASIS, 485 forms, and verbal orders promptly and accurately
  • Participate in chart audits and quality improvement activities
  • Maintain accurate clinical and patient records in compliance with agency, state, and federal guidelines
  • Provide age-appropriate care based on knowledge of growth and development principles

Patient Population and Common Diagnoses

  • Patient types include those requiring wound care, COPD, CHF, diabetes management, total joint care, complex assessments, medication management, IV medications, and PICC line care
  • Average census: 165 patients, 6 units per day

Required Qualifications

  • Valid Registered Nurse (RN) license in Wisconsin
  • Minimum 2 years of hospice nursing experience, including hospice experience within the last year
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
  • Home health nursing experience

Skills and Experience

  • Home health experience preferred
  • Experience with Epic electronic medical records system preferred
  • Excellent communication and interpersonal skills; ability to work respectfully with diverse clients
  • Strong organizational, analytical, and problem-solving abilities
  • Ability to collaborate effectively with physicians and other healthcare professionals
  • Proficient clinical skills with sound judgment and ability to work under direction
  • Competent in using necessary equipment such as fax, computer, copier, and telephone
  • Ability to assess and interpret patient data to meet individual care needs
  • Cultural sensitivity and appreciation for diversity among patients and families
