Description
Job Opportunity: Case Manager (Registered Nurse)
Location
- Bel Air, Maryland
Employment Type
- Travel/Contract
- Contract Length: 13 weeks
Schedule
- Monday through Friday, Day Shift (5x8 hours)
- Shift hours: 8:00 or 8:30 AM to 4:00 or 4:30 PM (depending on needs)
- Holidays required as scheduled
- Floating required between two nearby facilities within a 10-mile radius
- Weekends not required but available if interested
- On-call not required
Job Description
- Inpatient case management focusing on coordinating medical home care, placements, social situations, and safe discharges
- Oversees guardianships, complex medical cases, and social wrap-around services
- Average daily caseload: approximately 15 patients
- EPIC charting experience required
- Business casual attire
Required Qualifications
- Valid Registered Nurse (RN) license in Maryland
- Associate of Science in Nursing (ASN) / Associate Degree in Nursing (ADN)
- Minimum 3 years of case management experience, including within the last year
- Experience in inpatient and acute care settings
- Experience with EPIC electronic health record system
