Physical Therapist - Acute Care Hospital
Location: Cottonwood, Arizona
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Details
- Acute Care Physical Therapist role in a short-term acute care hospital (87 staffed beds) with teaching status and Level IV trauma designation.
- Shift: Tuesday through Saturday, 8:00 AM to 5:00 PM (5 days x 8 hours).
- Average daily patient census: 11.
- Guaranteed 40 hours per week; facility may call off up to 6 times during the 13-week assignment.
- Weekend Requirement: Must work at least one weekend day.
- Floating: May be asked to float to surrounding clinics.
- Dress Code: Business casual.
- Charting system: Cerner.
- Parking: Free.
- No on-call required.
- Locals accepted.
- Return-to-Office (RTO) restrictions require unit manager approval.
Required Qualifications
- Valid Physical Therapist license in Arizona.
- Minimum 1 year of Physical Therapist experience, with experience within the last year.
- BLS certification issued by the American Heart Association.
Compliance & Documentation Requirements
- Valid Arizona Driver’s License or State ID (passport alone not accepted).
- Background checks including 7-year history, SSN trace, nationwide criminal search, sex offender search, EPLS/GSA/SAM, and HHS/OIG checks required within 30 days of start.
- Fingerprinting: Current and valid Arizona fingerprint card required for Physical Therapists in acute care.
- Employment application with 3 years of work history; gaps over 30 days must be documented with city and state.
- Two professional references required (one from most recent employer within 1 year, second within 2 years).
- Physical exam within 1 year confirming ability to perform essential job functions.
- Drug screening: 10-panel urine test within 30 days of start, updated annually.
- Immunizations and health screenings:
- Negative TB test or chest x-ray within 1 year, updated annually.
- Tdap vaccine within last 10 years or declination.
- Measles, Mumps, Rubella: Positive titer or proof of 2 MMR vaccines.
- Varicella: Positive titer or proof of 2 vaccines.
- Hepatitis B: Positive titer, 3-dose vaccine series, or declination.
- COVID-19: Initial vaccine series or declination with facility-specific form.
- Seasonal Flu vaccine or signed declination form (mask required if declined).
- Fit mask (N95) test within 1 year, updated annually; PAPR not an option. Facility may provide onsite fit testing if needed.
Additional Requirements
- Must complete facility-specific job description form prior to start.
- Must review and sign facility policies including:
- Workforce member confidentiality agreement
- Substance abuse screening policy
- Professional attire and appearance policy
- Security question form (signed at least one week prior to start)
- Travel information verification form
- Medication test/pharmacology competency exam required within 1 year of start, minimum passing score 80%.
- Color blindness test required for all candidates starting 1/19/26, must pass with 100%.
- Core competency exams (Parts I, II, III) must be updated annually with minimum passing score of 80%.
Job Environment & Expectations
- Department operates 7 days a week; flexibility to work weekend shifts is mandatory.
- Ability to work with ventilated patients and manage moderate to maximum assistance patients is preferred.
- Must be adaptable to changing patient needs and able to pick up additional evaluations as needed.
- Adherence to quality and patient safety standards including medication administration protocols (BCMA) is expected.
This position offers an opportunity to work in a teaching hospital with a trauma designation, providing acute care physical therapy services in a dynamic environment requiring flexibility and strong clinical skills.
