Description
Medical Surgical Nurse (LVN / LPN) - Travel/Contract
Location: Rochester, New York
Contract Length: 13 weeks
Shift: Day (3 shifts per week, 12 hours each, 7:00 AM - 7:00 PM)
Hours per Week: 36
Unit & Work Environment
- 14-bed overflow med/surg telemetry unit for ED boarder patients
- Flex space within GI Endoscopy area; patient census may fluctuate based on demand
- Float required to care for med/surg patients based on census
- Weekend coverage every other weekend (EOW)
- Call requirements depend on staffing needs
- Communication via SBAR for ED patients; verbal report for other transfers/admissions
- Navy blue/white scrubs required
Required Qualifications
- Valid Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) license in New York
- Minimum 1 year of Long Term Care experience within the last year
- Med Surg nursing experience
- Experience with Epic electronic health record system
- Basic Life Support (BLS) certification issued by the American Heart Association
Candidate Requirements & Preferences
- No local candidates accepted within 75 miles
- Pending license not accepted
- Minimal return-to-office (RTO) restrictions
- Open to block scheduling but not guaranteed
- First-time travelers accepted
Compliance & Documentation Requirements
- Government-issued photo ID required
- 7-year criminal background check (including felony, SSN trace, national criminal, sex offender, education verification) within 3 months of start
- EPLS/GSA/SAM and HHS/OIG checks required within 3 months and updated monthly
- Foreign equivalency education evaluation if applicable
- Education verification required (hard copies of degrees/transcripts not accepted)
- Physical exam within 1 year of start, updated annually
- Fit mask test within 1 year, updated annually (specific mask types accepted)
- Negative TB test or chest X-ray within 1 year; TB questionnaire accepted in lieu of annual renewal
- Immunizations or titers required for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B
- Varicella
- Drug screening within 30 days of start (includes amphetamines, cocaine, fentanyl, heroin, opiates, etc.)
- Seasonal flu vaccination or signed declination required during flu season
- Snellen eye exam within 1 year of start
- COVID vaccination copy preferred but not required
- References (2 within 3 years) required for direct hire candidates
- Employment application and skills checklist required annually
- Work history verification of at least 2 past employments required for direct hire candidates
Additional Notes
- Facility uses SBAR communication method for ED patients
- LPNs are restricted from acting as resource or assuming assignments in GI or med/surg areas
- Float duties may include med/surg patients based on census
- Facility-specific onboarding modules and attestations required prior to start
- Fingerprinting and additional background checks may be required for certain units (mental health, behavioral health, home health aide, LTC)
