Description
Licensed Practical Nurse (LPN) – Medical Surgical
Location: Rochester, New York
Employment Type: Travel/Contract
Contract Length: 13 weeks
Shift: 4x12 hour shifts, rotating days and nights (7:00 AM–7:00 PM / 7:00 PM–7:00 AM)
Weekend Requirement: Every other weekend
Holiday Requirement: Available to work every other holiday, if needed
Required Qualifications
- Valid LPN/LVN license in New York
- Minimum 1 year of Long Term Care experience within the last year
- Medical-Surgical experience
- Basic Life Support (BLS) certification issued by the American Heart Association
- Experience with Epic electronic health record system
Clinical Skills & Responsibilities
- Able to take own patient assignment with RN support
- Vital signs, pain assessment, intake/output monitoring
- Isolation precautions, physical restraints, specimen labeling
- Oxygen administration via nasal cannula
- Feeding tube management
- Telemetry monitoring and 12-lead EKG
- Medication administration (oral, IM, SQ, NG/G-tube, ear and eye drops)
- IV therapy with pumps, blood glucose monitoring
- Basic wound care (simple dressings, JP/Hemovac drains)
- Bathing and mouth care
- Foley catheter insertion (male/female), clean catch urine collection
- Communicate status changes to RN/MD
- Experience in Medical-Surgical/Telemetry care setting
Unit & Work Environment
- Patient ratio: 1:5-6 (day shift)
- Patient population includes medical patients with COPD exacerbation, infections, failure to thrive, and social issues
- Floating within skillset required
- Uniform: Navy blue and white scrub colors
Additional Requirements
- Government-issued photo ID required
- 7-year criminal background check (including felony, SSN trace, national sex offender, education verification) within 3 months of start
- EPLS/GSA/SAM and HHS/OIG checks required within 3 months of start and run monthly
- Foreign education equivalency evaluation if applicable
- Education verification from primary source required (no hard copy degrees)
- Physical exam within 1 year of start, updated annually
- Fit mask (N95) fit testing within 1 year, updated annually
- Negative TB test or chest X-ray within 1 year; TB questionnaire accepted in lieu of annual renewal
- Immunizations or titers required for: Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella
- Drug screening within 30 days of start
- Seasonal flu vaccination or signed declination required during flu season
- Snellen eye exam within 1 year of start
- COVID vaccination copy preferred but not required
- Medication/pharmacology competency exam as required
- Color blindness testing required (not annually)
- Competency exams as required by specialty
- Orientation safety manual acknowledgement required
- Primary source verification of state license and NURSYS verification within 30 days of start
- References (2 within 3 years) required for travel candidates
- Employment application and work history verification required

- Locals accepted with adjusted pay rate
- Call-off policy allows up to three canceled shifts without penalty during contract
- Scheduling requests must be submitted upon application
