Occupational Therapist (Home Health)

Job Type

• Contractor
• Contract length: 13 weeks
• Shift: 8-hour day shift (approximately 7:45 AM – 4:00 PM), Monday through Friday
• No weekend, holiday, or on-call requirements

Location Coverage

• Primarily Racine and Kenosha Counties, Wisconsin
• Travel between patient homes within these counties; most travel times 20-30 minutes, occasionally up to an hour

Patient Population & Case Load

• Adult home health patients
• Common diagnoses: geriatrics, post-stroke, post-joint replacement, weakness, deconditioning, COPD, CHF
• Interdisciplinary team collaboration with RNs and PTs
• Morning team huddle via phone daily
• Caseload: approximately 6 patient visits (“points”) per day

  • Evaluation visits: 1.5 points
  • Standard visits: 1 point

Required Qualifications

• Minimum 3 years of Occupational Therapy experience (SNF, home health, or acute care; home health experience preferred)
• Accepting first-time travel therapists
• Valid Wisconsin OT license
• BLS certification
• Bachelor’s degree in Occupational Therapy accepted
• Active driver’s license, vehicle, and auto insurance required

Skills & Responsibilities

• General OT treatment and evaluation skills
• Focus on activities of daily living (ADLs) and adaptive equipment

Orientation & Training

• First day orientation at office location in Milwaukee
• EPIC electronic medical record (EMR) training included
• Preceptor-supported orientation for remainder of the week

Work Environment & Support

• Supported by RN supervisor, PT lead and manager, and other OTs
• Use of EPIC EMR system preferred
• Business casual or any color scrubs acceptable
• Parking available at patient homes

Additional Information

• Homecare experience is a plus
