Description

Job Opportunity: RN Hospice Supportive Care Team (Home Hospice)

Position Overview

  • Role involves providing hospice care in patients' homes.
  • Territory coverage varies by requisition but includes multiple locations within Wisconsin.
  • Patient types include those with failure to thrive, advanced cancers, end-stage COPD, terminal illness, and other end-stage disease processes.
  • Responsibilities include symptom management, fall assessments, death visits, and after-hours visits as needed (some issues may be resolved via phone).

Contract Details

  • Contract length: 13 weeks
  • Shift: 12-hour night shifts (8:00 PM to 8:00 AM)
  • Hours per week: 24 hours (typically 3 shifts per week)
  • Weekend rotation: Every other weekend
  • Holiday expectations: One summer and one winter holiday
  • Schedule: Usually set schedule; details discussed during second interview
  • No on-call requirement

Patient Ratios

  • RN caseload ranges from 0 to 6 patients, depending on call-ins during the night shift

Required Qualifications

  • Minimum 2 years of experience in Home Health or Home Hospice nursing
  • First-time travelers accepted if they have case management experience in the home setting
  • Active Wisconsin RN license
  • BLS certification
  • Valid driver’s license, own vehicle, and auto insurance

Skills and Competencies

  • Comfortable performing focused patient assessments and critical thinking
  • Skilled in investigating patient agitation causes (e.g., urinary issues, Foley catheter troubleshooting)
  • Experience with Foley catheter exchanges, feeding tube troubleshooting, wound care, and post-fall assessments
  • Comfortable conducting death visits and communicating with medical examiners or coroners
  • Ability to administer narcotic pain medications and identify potential drug diversions in the home setting

Orientation and Support

  • Orientation includes 3 shifts (1 week) with a Supportive Care Team nurse at the territory office
  • 24/7 access to a Lead RN for support and reporting after visits
  • Other hospice team members available via phone for assistance

Technology and Equipment

  • Electronic Medical Record system: Epic (Rover)
  • Occasional use of IV pump (Sapphire), though rare in home settings
  • Provided equipment includes badge, laptop, cell phone, and a trunk kit

  • May occasionally float slightly outside assigned territory if closer to a call, but never more than one hour away
