Schedule & Hours
- 36 hours per week total
- Shift pattern: 3 shifts of 8 hours and 1 shift of 12 hours per week
- Shift times vary: 11pm–7am (8 hrs), 3pm–11pm (8 hrs), 7pm–7am (12 hrs)
- Weekend coverage every other weekend
Unit & Patient Population
- Facility with 180 beds across 5 units
- Patient diagnoses include dementia, diabetes mellitus, cerebrovascular accident (CVA), cardiac conditions, orthopedic treatment, neurobehavioral issues, and long-term care needs
- Units include neurobehavioral (dementia/Alzheimer’s with violent behaviors), rehab (post-orthopedic surgery, cardiac procedures), secure dementia (elopement risk), and long-term care units
Role & Responsibilities
- Serve as a clinical resource and problem solver during off shifts (evenings, nights, weekends, holidays)
- Provide clinical expertise and facilitate patient flow
- Mentor and coach staff
- Supervise nursing care across all units
- Manage special equipment such as wound vacs, PICC lines, nephrostomy tubes, tube feeds, indwelling catheters, and colostomies
- Handle code management, de-escalation techniques, emergency preparedness, and managing assaultive behavior
- Ensure compliance with HIPAA, infection control, isolation procedures, and regulatory requirements (DPH, TJC, OSHA)
- Oversee documentation related to sentinel events, death of a patient, and risk management
- Support new admissions and centralized staffing
- Coach and counsel staff to meet performance standards
- Maintain workplace safety and productivity standards
- Provide PICC line care/management
Required Qualifications
- Valid Registered Nurse (RN) license in New York
- Minimum 2 years of House Supervisor experience, including experience within the last year
- Experience in charge nurse roles
- Basic Life Support (BLS) certification issued by the American Heart Association
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- Experience with Epic electronic health record system
- Proficient in code management
Orientation
- Orientation held during daytime hours in the first week
Certifications & Competencies
- BLS required
- Advanced certifications (ACLS, PALS, NRP, TNCC, EFM, NIH Stroke Certification) as required by unit description
- Medication/pharmacology competency exam as required
- Color blindness testing required once
- Orientation safety manual acknowledgement required
- State license primary source verification and NURSYS verification within 30 days of start
Background & Compliance Requirements
- Government-issued photo ID required
- 7-year criminal background check within 3 months of start (including SSN trace, national criminal, sex offender, education verification)
- EPLS/GSA/SAM and HHS/OIG checks required within 3 months and updated monthly
- Foreign education equivalency evaluation if applicable
- Education verification from primary source required (no hard copies)
- Employment application and work history verification required
- References required (2 within 3 years) for direct hires
- State-specific checks including NYS Nurse Aide Registry (if applicable), criminal background fingerprinting (if applicable), disciplinary action search, sex offender registry, and Medicaid exclusion list checks
Health & Immunization Requirements
- Physical exam within 1 year, updated annually
- Fit mask test within 1 year, updated annually (specific acceptable mask types)
- Negative TB test or chest X-ray within 1 year; TB questionnaire accepted in lieu of annual renewal
- Immunizations or positive titers required for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B (surface antibody positive titer, completed series, or declination)
- Varicella
- Drug screening within 30 days of start (includes amphetamines, cocaine, fentanyl, heroin, opiates, etc.)
- Seasonal flu vaccination or signed declination required during flu season
- Snellen eye exam within 1 year
- COVID vaccination copy preferred but not required
Additional Notes
- Scrubs: Any
- Candidates must complete all health-related requirements and lab work within 5 days of signing the contract
- Facility accepts first-time travel nurses but requires prior charge experience
- This role involves supervisory responsibilities during night shifts and requires strong clinical and leadership skills in a skilled nursing facility environment
