Description

Job Opportunity: Nursing Supportive Care Team (SCT) - Home Hospice Care

Position Details

  • Employment Type: Contractor
  • Contract Length: 17 weeks
  • Shift: 12-hour day shifts (8:00 AM – 8:00 PM), weekends only (Saturday and Sunday)
  • Hours per Week: 24 hours
  • Schedule: Weekend rotation every other weekend; typically a set schedule with 3 shifts per week
  • Holiday Expectation: 1 summer holiday and 1 winter holiday
  • On Call: None
  • Orientation: One week (3 shifts) with a Supportive Care Team nurse at the territory office

Work Environment

  • Home hospice nursing role requiring travel to patients’ homes within a defined territory in Wisconsin.
  • May occasionally float slightly outside assigned territory if needed, but never more than an hour away.
  • Collaborative team culture focused on timely patient care.
  • Provided equipment includes badge, laptop, cell phone, and trunk kit.
  • Free parking available at territory office.
  • Scrub color: Navy blue.

Patient Population & Care

  • Patient types include those with failure to thrive, advanced cancers, end-stage COPD, terminal illness, and other end-stage disease processes.
  • Responsibilities include symptom management, fall assessments, death visits, and after-hours triage calls (some resolved by phone).
  • Patient ratio for RNs ranges from 0 to 6 depending on call-ins.

Required Qualifications

  • Valid RN license in Wisconsin
  • Associate of Science in Nursing / Associate Degree in Nursing
  • Minimum 2 years of Home Health experience, including experience within the last year
  • Case management experience in the home setting
  • Hospice experience
  • Current BLS certification
  • Experience troubleshooting feeding tubes
