Registered Nurse - Home Health (South Carolina)
Job Details
- Location: Clinton, South Carolina
- Employment Type: Travel/Contract
- Contract Length: 13 weeks
- Shift: Day shift, Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours)
- Average Patients per Day: 5-8
- Weekly Travel: Approximately 300-500 miles, traveling up to 75 miles from home clinic
- Work Setting: Patient homes and Assisted Living Facilities (ALF)
- EMR Systems: HomeCare HomeBase (HCHB), OASIS, Outlook
- On-Call: Required 1-2 days during the week and 1 weekend per month (varies)
- Weekend Requirement: Varies; discuss during interview
- Dress Code: Scrubs
Job Description
- Provide and coordinate home nursing care according to physician orders and agency policies.
- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Manage cases including Start of Care, Wound Care, and Follow-up Visits.
- Must have reliable transportation, a current driver’s license, and agency-required liability insurance.
- Proficiency in HomeCare HomeBase required within the last 6 months; retraining provided if needed.
- RNs must be proficient in OASIS documentation.
Required Qualifications
- Valid Registered Nurse (RN) license in South Carolina.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
- Minimum 1 year of recent experience in Home Health, including experience with adult patients.
- Minimum 1 year of Hospice experience within the last year.
- Current BLS certification issued by the American Heart Association or American Red Cross.
- Experience working with HomeCare HomeBase EMR system.
Productivity Expectations
- Weekly productivity units: 25-30 units
Additional Requirements & Compliance
- Must complete a standard 10-panel drug screen.
- Physical exam within 12 months prior to start, confirming good health and no communicable diseases.
- Negative TB test or chest X-ray within 12 months, updated annually.
- Immunization or titer proof for Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella, and COVID-19 (or documented declination).
- Seasonal flu vaccination or facility-specific declination required.
- Two professional references required.
- Verification of at least two most recent employers or last 7 years of work history.
- Must have personal or auto insurance policy under candidate’s name or be listed as a driver on the policy.
Background and Screening Requirements
- Comprehensive background check covering the last 7 years, including SS trace, county, nationwide, sex offender, and state-specific searches.
- State-specific fingerprinting and registry checks as required by South Carolina and other applicable states.
- Monthly EPLS/GSA/SAM and HHS/OIG screenings.
- Completion of various compliance documents including confidentiality, HIPAA, privacy policies, and agency-specific compliance training prior to start.
- Must sign all required agreements and attestations before starting the assignment.
Notes
- Local candidates accepted at the same rate.
- Schedule may vary; some positions may follow a 4x10 schedule (Friday-Monday), but most are standard 5x8.
- If patient census is low, floating to nearby clinics may be arranged to maintain hours.
- Candidates must be willing to travel within the assigned territory.
This position is ideal for experienced RNs with recent home health or hospice experience who are comfortable working independently in patient homes and managing a moderate patient load with travel requirements.
