Medical Surgical LPN Travel/Contract Position

Location: Rochester, New York
Contract Length: 13 weeks
Shift: Nights, 4 shifts per week, 12 hours each (7:00 PM – 7:00 AM)
Weekend Requirement: Every other weekend
Holiday Requirement: Must be available to work every other holiday, if needed

Required Qualifications

• Valid LPN / LVN license in New York
• Minimum of 1 year of Long Term Care experience within the last year
• Medical-Surgical nursing experience
• Experience with Epic electronic health record system

Clinical Skills & Responsibilities

• Able to take own patient assignment with RN support
• Blood product administration
• Central line, peripheral line, PICC line care/management
• Phlebotomy and IV start/maintenance
• High Flow Nasal Cannula (HFNC), nasal cannula, oxygen administration, oxygen tanks
• Tracheostomy care/management
• Feeding tube care/management
• Heparin protocols
• Pacemaker (temporary and permanent) monitoring
• Telemetry monitoring on the unit
• Use of rapid response/code teams
• Isolation precautions
• Wound care including wound vac
• Care for orthopedic trauma, total joint replacements, surgical drains

Unit & Work Environment

• Patient ratio: 1:5–6
• Unit serves complex care patients including medicine, orthopedic overflow, and complex long-term patients
• Floating within skillset required
• Uniform: Navy blue and white scrub colors

Scheduling & Policies

• Must submit requests for time off, block, and special scheduling upon application
• Client may cancel up to three shifts during the 13-week contract without penalty
• Locals accepted with a pay adjustment
• First-time travelers accepted

Required Documentation & Compliance

• Government-issued photo ID required
• 7-year criminal background check within 3 months of start (including SSN trace, national criminal, sex offender, education verification)
• EPLS/GSA/SAM and HHS/OIG checks required within 3 months and run monthly
• Foreign education equivalency evaluation if applicable
• Education verification from primary source required (not hard copies)
• Physical exam within 1 year, updated annually
• Fit mask test within 1 year, updated annually (specific mask types accepted)
• Negative TB test or chest X-ray within 1 year; TB questionnaire accepted in lieu of annual renewal
• Immunizations or positive titers required for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (surface antibody positive titer, completed series, or declination)
  • Varicella
• Drug screen within 30 days of start (includes amphetamines, cocaine, fentanyl, heroin, opiates, etc.)
• Seasonal flu vaccination or signed declination required during flu season
• Snellen eye exam within 1 year
• COVID vaccination copy preferred but not required

Additional Certifications (As Required by Unit)

• ACLS (AHA provider/instructor/RQI accepted)
• PALS
• NRP
• TNCC
• EFM
• NIH Stroke Certification
• Orientation Safety Manual Acknowledgement
• Medication/Pharmacology competency exams
• Color blindness testing (one-time)
• Competency exams as required by specialty

Verification & Onboarding

• Employment application required
• Work history verification (at least two past employments for direct hires)
• References required for direct hires (2 references within 3 years)
• Photo for badge (passport-style with solid background)
• Facility-specific onboarding modules and attestations to be completed

State-Specific Requirements (New York)

• NYS Nurse Aide Registry required within 3 months for certain units (LTC, Home Care, Hospice)
• Fingerprinting and criminal background check fingerprint results may be required for Mental Health, Behavioral Health, Home Health Aide, and LTC units
• Disciplinary action search results required for certain units
• Sex offender registry search required within 30 days of start
• NY Medicaid Exclusion List check required within 3 months and run monthly

This position is suitable for LPNs with recent acute care experience who are comfortable working night shifts in a complex medical-surgical environment with telemetry monitoring and a variety of advanced clinical skills. First-time travel nurses are welcome to apply.
