Job Opportunity: Home Care Physical Therapist

Position Details

• Employment Type: Contractor
• Contract Length: 13 weeks
• Shift: 8-hour day shift, Monday through Friday
• Hours per Week: 40
• Work Location:

  • Orientation onsite for approximately 2 days in Oak Brook, IL
  • After orientation, position is 100% remote within the designated Central Metro East coverage area (Chicago area zip codes including 60630, 60618, 60625, 60646, 60659, 60660, 60657, 60640, 60613, 60614, and surrounding areas)
  • No physical office; all meetings held virtually
• Patient Census: Approximately 200 adult/geriatric general medical/surgical patients
• Patient Visits: Expectation to complete 6 patient visits per day within coverage area
• Transportation: Must provide own reliable transportation to/from all patient visits
• Attire: Any solid color scrubs or business casual; no jeans or open-toe shoes

Required Qualifications

• License: Active Illinois Physical Therapist license
• Certifications: BLS (AHA) certification
• Experience: Minimum 2 years of recent home health physical therapy experience with adult/geriatric patients
• Driver Requirements: Valid driver’s license and auto insurance
• Technical Skills:

  • Ability to assist patients with Activities of Daily Living (ADLs) and adaptive equipment
  • Effective collaboration with other specialties (e.g., RN, OT, Speech Therapy)

Additional Information

• Orientation: Includes EMR training and skills check-off
• Electronic Medical Record (EMR): EPIC
• Scheduling:

  • No weekend rotation
  • No on-call or holiday work required
• Support: Nursing supervisors, managers, and team leads available for support
• Floating: May be required within the specified coverage area
• Travel Experience: First-time travelers are accepted if the candidate is qualified
• Patient Ratio: 1:1, with 6 patients per day expected
