Description
Job Opportunity: Home Care Physical Therapist
Position Details
- Employment Type: Contractor
- Contract Length: 13 weeks
- Shift: 8-hour day shift, Monday through Friday
- Hours per Week: 40
- Work Location:
- Initial orientation onsite for approximately 2 days
- Position is then 100% remote within a designated coverage area
- No physical office; all meetings held virtually
- Coverage Area: Central Metro East area of Chicago, IL (specific zip codes include 60630, 60618, 60625, 60646, 60659, 60660, 60657, 60640, 60613, 60614, and surrounding areas)
- Patient Census: Approximately 200 adult/geriatric general medical/surgical patients
- Patient Visits: Expectation to complete 6 patient visits per day within the coverage area
- Transportation: Candidate must provide their own reliable transportation to/from all patient visits
- Attire: Any solid color scrubs or business casual; no jeans or open-toe shoes
Required Qualifications
- Valid Physical Therapist license in Illinois
- Minimum 2 years of Physical Therapist experience, including experience within the last year
- BLS certification issued by the American Heart Association
- Master's degree in Occupational Therapy or Master's degree in Physical Therapy
- Experience working with adult or geriatric patients
- Experience in a Home Health setting
- Experience with Epic electronic medical record system
- Experience with OASIS documentation
Additional Information
- Orientation: Includes EMR training and skills check-off
- Electronic Medical Record System: EPIC
- Equipment: Use of Sapphire IV pumps (medication dispensing and monitors not applicable)
- Scheduling:
- No weekend rotation
- No on-call duties
- No holiday work required
- Support: Nursing supervisors, managers, and team leads available for support
- Floating: May be required within the specified coverage area
Notes
- Candidates with 1 year of recent Home Health experience may be considered if highly qualified
