Description
Registered Nurse - Hospice Case Manager
Location: Barre, Vermont
Employment Type: Contract
Contract Length: 20 weeks
Work Schedule: Monday - Friday, 36 hours per week
Job Description
- Provide hospice care in a home setting as part of hospice case management.
- Experience with hospice case management is required.
- Prior experience with rural travel is preferred.
- Experience using McKesson/Netsmart EMR systems is preferred.
- Preference for candidates without Return to Office (RTO) requests for this contract.
Required Qualifications
- Valid Registered Nurse (RN) license in Vermont.
- Minimum of 1 year of case management experience, including experience within the last year.
- Basic Life Support (BLS) certification.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
- Experience in a home health setting.
- Hospice care experience.
Additional Information
- This position involves travel to rural areas.
- The role supports patients receiving hospice care in their homes.
