Job Description

• Provide nursing services to individuals and families in their residence under the supervision of a registered nurse.
• Perform prescribed individualized patient care as delegated.
• Assist the RN in preparing, evaluating, and modifying the patient's plan of care as appropriate.
• Observe, document, and report the patient's physical and emotional needs and reactions to care to the RN.
• Work as part of a cohesive patient-care team with broad independence.

Required Qualifications

• Valid Registered Nurse (RN) license in Vermont.
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
• Minimum 2 years of experience in a Home Health setting, including experience within the last year.
