The LPN will:

<ul>
	<li>Provide nursing services to individuals and families in their residence under the supervision of a registered nurse.
	</li><li>Perform prescribed individualized patient care to patients as delegated.
	</li><li>Assist the RN in the preparation, evaluation, and modification of the patient's plan of care as appropriate.
	</li><li>Observe, document, and report to the RN as appropriate, the patient's physical and emotional needs and reactions to care.
</li></ul>

Minimum Qualifications:

<ul>
	<li>Must be a Licensed Practical Nurse in Vermont and New Hampshire or hold an unencumbered compact license.
	</li><li>A graduate from an approved school of practical nursing.
	</li><li>2 years experience in home health setting.
