Physical Therapist - Home Health (Travel/Contract)
Location: Roseburg, Oregon
Contract Length: 13 weeks
Job Overview
- Provide physical therapy services in a home health setting with home visits and periodic office meetings.
- Average caseload: 5-6 patients per day, with 5-10 patients assigned daily equating to roughly 6-8 points per day (30 points expected weekly).
- Types of cases include Start of Care and Follow Ups.
- Must be willing to travel up to 75 miles from the home clinic, averaging about 100 miles traveled per week.
- Schedule typically Monday through Friday, 8:00 AM to 5:00 PM (5x8 hours). Some positions may have a 4x10 schedule (Friday-Monday). Weekend requirements vary and should be discussed during the interview.
- No on-call duties required for this position.
Required Qualifications
- Valid Physical Therapist license in Oregon.
- Minimum of 1 year of Physical Therapist experience within the last year.
- Experience in a Home Health setting.
- BLS certification required.
Productivity Expectations
- Weekly productivity target: 30-32 points.
- Documentation and travel time are factored into point system.
Electronic Medical Records (EMR)
- Experience with PointCare and Homecare Homebase (HCHB) EMR systems is required.
Additional Requirements & Compliance
- Must complete and maintain all required background checks, including but not limited to:
- 7-year background check covering all locations lived/worked/schooled.
- EPLS/GSA/SAM and HHS/OIG screenings.
- Fingerprinting as required by state regulations.
- State-specific background and registry checks as applicable.
- Required health documents include:
- Physical exam within 12 months prior to start.
- Negative TB test or chest X-ray within 12 months.
- Proof of immunizations or titers for Measles, Mumps, Rubella, Varicella, Hepatitis B.
- Drug screening (10-panel).
- Seasonal flu vaccination or documented declination.
- COVID-19 vaccination series or documented declination.
- Must review and sign various compliance, confidentiality, privacy, and policy documents prior to start and annually thereafter if working consecutively for more than one year.
- Personal or auto insurance policy required, must be under candidate’s name or list candidate as a driver.
References & Work History
- Two professional references required.
- Verification of at least two most recent employers or last seven years of work history.
Notes
- Local candidates are accepted at the same rate as travelers.
- Mileage reimbursement follows IRS standard rate; compensation is from patient home to patient home, not including commute to/from first and last patient.
- Weekend and on-call requirements vary by location and should be clarified during the interview process.
- Candidates must be prepared to complete all required documentation and compliance training prior to assignment start.
