PT Home Health - Travel/Contract
Location: Portland, Oregon
Contract Length: 13 weeks
Job Details
- Setting: Home Health
- Shift: Day shift, 8 hours per day (5x8 hour shifts, 8:00 AM - 4:30 PM)
- Patient Load: 1:1 ratio, up to 6 patients per day
- Productivity Standard: 30 productivity points per day
- Weekend Work: Not required
- On-Call: Not required
- Floating: Not required
- Block Schedule: Not accommodated
- Locals Accepted: Yes, within 50-75 mile radius
- Transportation: Traveler must have own transportation; daily travel radius up to 60 miles (60-90 minutes total drive time)
- Charting System: EPIC Remote Client (laptop), Rover (phone app), and Oasis
- Attire: Business casual; scrubs allowed but no specific color required; no jeans
- Parking: Free
Required Qualifications
- Valid Physical Therapist license in Oregon
- Minimum 1 year of Physical Therapist experience, including experience within the last year
- Experience in a Home Health setting
- Experience with OASIS documentation
- BLS (Basic Life Support) certification
Health & Safety Requirements
- Physical Exam: Within 1 year of start, signed by MD, NP, PA, or DO, indicating fitness to work
- Immunizations/Vaccines:
- TDAP (Tetanus/Diphtheria/Pertussis) within 10 years or declination accepted
- MMR (Measles, Mumps, Rubella): 2 vaccines or positive titer or declination
- Varicella: 2 vaccines or positive titer or declination
- Hepatitis B: Documentation of vaccination series or signed declination
- Flu Vaccine: Required during flu season (9/1–3/30) or declination accepted
- COVID-19: Vaccination record preferred; declinations not required but unvaccinated must follow masking policies
- TB Test: Negative IGRA/TSPOT, QGold, or 2 PPDs within 12 months; must be updated annually
- Drug Screen: 9-panel drug screen within 12 months of start; instant tests acceptable
- Respirator Clearance: Required for travel candidates, medically cleared annually via RespSafety questionnaire
- Fit Mask Test: Required for travel candidates, updated annually; onsite testing available if needed
Background & Compliance
- Background Check: Within 90 days of start, covering all locations lived/worked in past 7 years; includes SSN trace, death master index, nationwide checks, MVR (required for home health), and state-specific abuse/neglect searches as applicable
- State-Specific Searches: Required for Oregon and Washington as applicable, including WA WATCH and OBRA Registry for CNAs (not applicable here)
- Education Verification: Required for all candidates to verify relevant education
- NURSYS Verification: Required for all RNs (not applicable for PT)
- References and Work History Verification: Required as noted above
- Photo for Badge: Passport-style photo with neutral background required
Additional Notes
- Guaranteed Hours: Facility may call off up to one shift per pay period (every two weeks)
- Return to Office (RTO) Restrictions: Confirm with hiring manager
- Float Policy: Not required for this position
- System groups clients to minimize drive time
- Annual Regulatory and Specialty-Specific Training Modules: Not required for this home health position at this facility
