Job Opportunity: Occupational Therapist - Acute Care Hospital

Location

  • Cottonwood, Arizona

Employment Type

  • Travel/Contract
  • Contract Length: 9 weeks

Work Schedule

  • Sunday through Thursday, 5 days a week, 8-hour shifts (7:30 AM to 4:00 PM)
  • 40 hours per week guaranteed
  • Weekend Requirement: Must work at least one weekend day during the assignment
  • Blocked scheduling in place
  • Facility may call off up to 6 times during the 13-week assignment period

Job Description & Responsibilities

  • Inpatient Occupational Therapist role
  • Preferred generalist comfortable with medical diagnoses
  • Ability to manage moderate to maximum assistance patients
  • Experience with co-treatments and ventilated patients preferred
  • Flexibility to pivot and take on additional evaluations throughout the day

Required Qualifications

  • Valid Occupational Therapist license in Arizona
  • Minimum 1 year of Occupational Therapy experience within the last year
  • Basic Life Support (BLS) certification issued by the American Heart Association (AHA)
  • Must be able to work at least one weekend day during the assignment

Additional Requirements

  • No on-call or floating required
  • Unit manager approval required for return to office (RTO) requests
  • Locals are accepted for this position

Compliance & Documentation

  • Employment application required with 3 years of work history; gaps over 30 days must be documented with city and state
  • Two professional references required (one from most recent employer within 1 year)
  • Background checks including 7-year criminal search, SSN trace, sex offender search, EPLS/GSA/SAM, HHS/OIG within 30 days of start
  • Education verification required (diploma or transcripts acceptable if verification pending)
  • Fingerprinting required for Arizona license (fingerprint card or application number prior to start)
  • Drug testing: 10-panel within 30 days of start, updated annually
  • Physical exam within 1 year confirming ability to perform essential job functions
  • Fit mask (N95) required within 1 year and updated annually; facility may provide onsite fit testing if needed
  • Immunizations and titers required or declination forms accepted for:
    • Tetanus/Diphtheria/Tdap (preferred within last 10 years)
    • Measles (Rubeola), Mumps, Rubella (proof of immunity or vaccines)
    • Hepatitis B (titer, vaccine series, or declination)
    • Varicella (proof of immunity or vaccines)
    • Negative TB test or chest X-ray within 1 year, updated annually; TB questionnaire required annually if past positive
    • Seasonal flu vaccination or facility-specific declination form (mask required if declined)
    • COVID vaccination series or declination with facility-specific form

Policies & Agreements

  • Must review and sign facility-specific policies prior to start, including:
    • Workforce member confidentiality agreement
    • Substance abuse screening policy
    • Professional attire and appearance policy
    • Security question form (signed at least one week prior to start)
    • Travel information verification form

Notes

  • No prior travel experience required; first-time travelers accepted
  • No on-call or floating duties
  • Facility adheres to quality and patient safety standards, including barcode medication administration (BCMA)
