PT Home Health - Medford, OR
Job Details
- Location: Medford, Oregon (and surrounding areas)
- Employment Type: Travel/Contract
- Contract Length: 13 weeks
- Shift: Day shift, 8 hours per day (8:00 AM - 4:30 PM), 5 days a week (40 hours/week)
- Schedule: No weekends required; no on-call; no floating between units; no block schedules
- Patient Load: 1:1 ratio, up to 6 patients per day; 30 productivity points standard
- Local Candidates: Accepted within a 50-75 mile radius; locals paid the same rate
- Start Date: ASAP
- Guaranteed Hours: Facility may call off up to one shift every two weeks
- Transportation: Traveler must have their own transportation; occasional travel radius up to 50+ miles
- Attire: Any color scrubs
- Charting System: EPIC/Oasis
- Parking: Free
- Mileage Reimbursement: IRS standard
Required Qualifications
- Valid Physical Therapist license in Oregon (pending license accepted)
- Minimum 1 year of Physical Therapist experience within the last year
- Experience in a Home Health setting
- Experience with OASIS documentation
- Basic Life Support (BLS) certification
