Description

RN Case Manager, Home Health

Location: Northampton, Massachusetts
Employment Type: Long-Term Assignment (13 weeks)
Shift: Monday – Friday, 8:00 AM – 4:30 PM with 1 in 4 weekend rotation


Job Description

  • Provide skilled nursing care and case management to patients and families in their homes.
  • Work under the direction of the Clinical Team Manager following organizational policies and the State Nurse Practice Act.
  • Utilize nursing theory and processes to assess, document, and communicate patient observations.
  • Promote and maintain patient health through direct care, teaching, counseling, and rehabilitative measures.

Required Qualifications

  • Valid Registered Nurse (RN) license in Massachusetts.
  • Minimum 1 year of Home Health nursing experience within the last year.
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).

Required Credentials & Compliance (Must be current and verifiable)

  • Professional license verification within 30 days of start date; updated upon expiration.
  • Fit testing for N95 respirator within 365 days of start date; updated annually (no exemptions allowed).
  • Respirator Medical Clearance attestation if fit test is required.
  • Documentation of immunizations: MMR (measles, mumps, rubella) or proof of immunity.
  • TB screening: Negative TB skin test or blood test within 3 months of start date; updated if break >6 months.
  • Physician health statement within 365 days of start date; updated if break >6 months.
  • Criminal background check within 45 days prior to start; includes county and international residence history for last 7 years.
  • Massachusetts CORI background check within 30 days prior to start.
  • National sex offender registry check within 30 days prior to start; updated annually.
  • Exclusion checks (OIG, GSA, MassHealth, NH Medicaid) within 30 days prior to start; updated annually.
  • Motor Vehicle Record (MVR) check within 45 days prior to start (required for all home care positions).
  • Submission of a recent color photo (taken within last 6 months) meeting specified criteria.
  • Verification of highest education level achieved.
  • Resume including past 7 years of work experience with city and state.
  • Two professional references within 4 years, including at least one supervisor or higher.
  • Completion of skills checklist within 1 year of submission.
  • Candidate submission coversheet for initial assignment.

Additional Information

  • Must have a valid driver’s license or state ID.
  • Assignment confirmation and Point of Interest (POI) form required upon acceptance and for any start date changes.
  • Positive TB test requires documentation of a negative chest x-ray after the positive test.
  • Name change documentation required if legal name differs from credential documents.

This position requires adherence to all credentialing and compliance standards prior to and during the assignment.
