Care Coordinator (Registered Nurse) - Travel/Contract
Location: Paoli, Pennsylvania
Contract Length: 13 weeks
Job Summary
- Role: Care Coordinator in Social Services
- Facility Type: Acute Care, Level II Teaching Hospital
- Total Staffed Beds: 261
- Shift: Day shift, 5 days a week (8:00 AM - 4:30 PM)
- Hours: 40 hours per week, 8 hours per day
- Patient Ratios: 15-20 patients per nurse
- Weekend Requirement: No mandatory weekends; optional weekend work available
- On-call: Not required
- Charting System: Epic
- Scrub Color: Wine-colored scrubs preferred but not mandatory
Required Qualifications
- Valid Registered Nurse (RN) license in Pennsylvania
- Minimum of 1 year of case management experience, including within the last year
- Completion of at least one prior travel assignment
- BLS certification issued by the American Heart Association or American Red Cross
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- Experience as a Care Manager or in case management roles
Additional Requirements
- Background Checks:
- Criminal background check within 6 months covering all locations lived/worked/schooled in past 7 years
- EPLS/GSA/SAM and HHS/OIG checks within 30 days of start, updated monthly
- Education Verification: Diploma, degree, or transcripts required
- Health Documents:
- Physical exam or statement of good health within 1 year prior to assignment, updated annually
- Fit mask test within 1 year, updated annually (specific approved masks required)
- Negative TB test (QuantiFERON Gold or T-Spot) within 6 months prior to assignment
- Immunizations: MMR (Measles, Mumps, Rubella), Varicella, Hepatitis B, Tdap vaccine
- Drug screening: 10-panel drug screen within 45 days of start, updated annually
- Flu vaccine or approved exemption during flu season (Sept-April 1)
- Respirator clearance within 1 year, updated annually

- Pennsylvania Department of Aging Fingerprints
- Pennsylvania Department of Human Services Fingerprint Results
- Pennsylvania PATCH Search Results
- References: 3 required (most recent manager, previous manager, peer)

- Orientation Safety Manual Acknowledgement required
- Confidentiality Agreement must be reviewed and signed
- Proof of valid government-issued photo ID and driver's license without restrictions required

- Return to Office (RTO) restrictions are at manager's discretion
