Description

Registered Nurse (Home Health) – Vermont

Job Type

36 hours per week
Day shift: Monday to Friday with weekend rotation approximately every 4th week

Location

Barre, Vermont

Job Description

Provide individualized, goal-oriented nursing care using the nursing process in accordance with physician orders.
Serve as Case Manager when acting as the lead discipline in cases.
Deliver home health and hospice care focused on holistic, community-centered support.

Required Qualifications

Valid RN license for Vermont.
BLS/BCLS certification.

Contract Length

12 weeks
