Description

American Traveler is hiring an experienced RN Case Manager for a Home Health position in Vermont and New Hampshire, requiring a minimum of 2 years of home health experience and an active VT or compact RN license.

Details

Home health setting covering more than 70 towns across Vermont and New Hampshire, including the Bradford, Randolph, Springfield, White River Junction, and Lebanon areas
Services provided include nursing, rehabilitation, hospice, and personal care
Full-time, day shift schedule
13-week contract

Requirements

Active RN license in Vermont and New Hampshire, or an unencumbered compact RN license
Current BLS/BCLS certification required
Minimum 2 years of home health nursing experience
Graduate of an accredited school of nursing

Additional Information

Develop and periodically review individualized patient care plans in collaboration with physicians, patients, families, and primary caregivers
Coordinate patient care through interdisciplinary communication and case conferences, advising other disciplines of discharges in a timely manner
Provide ongoing assessment of patients' clinical, functional, and emotional status, communicating significant changes to the physician and VNA colleagues
Teach patients and caregivers self-care techniques, including medication management, diet instructions, and health counseling as ordered by the physician
Work with broad independence while remaining an integral part of a cohesive interdisciplinary care team
