Description
Registered Nurse - Home Health
Location: Dallas, Massachusetts
Contract Length: 13 weeks
Shift: Monday to Friday, Day shift, No on-call or weekend work
Job Description
- Provide home health nursing care within a coverage area including multiple towns within approximately a 60-mile radius.
- May be asked to assist in other territories within the service area as needed.
- Documentation requirements include:
- Start of Care visits (2.5 points)
- Resumption of Care visits (2 points)
- Recertifications (1.75 points)
- Discharges (1.5 points)
- Routine visits (0.9 points)
- Must achieve a minimum of 30 points.
Required Qualifications
- Valid Registered Nurse (RN) license in Massachusetts.
- Minimum of one year of experience in a Home Health setting, with experience within the last year preferred.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
- Experience with HomeCare HomeBase (HCHB) electronic medical records system.
Additional Information
- Overtime requires prior written approval from the Administrator.
- Two weeks cancellation notice required.
