Job Opportunity: Registered Nurse - Home Health

Job Description

  • Provide and coordinate home nursing care according to physician orders and agency policies.
  • Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
  • Conduct home visits with periodic office meetings.
  • Average patient load: 5-8 patients per day.
  • Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
  • Must be willing to travel up to 75 miles from the home clinic; approximately 400 miles traveled weekly.
  • Use of HomeCare HomeBase (HCHB) EMR required; proficiency in OASIS is required for RNs.
  • Typical schedule: Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours), with some positions offering 4x10 shifts (Friday-Monday).
  • Weekend rotation: approximately one weekend per month.
  • On-call requirement: one day per week plus the weekend worked.

Required Qualifications

  • Valid RN license in Arizona.
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
  • Minimum 1 year of recent Home Health nursing experience within the last year.
  • Experience working with adult patients in a Home Health setting.
  • Hospice experience.
  • Current Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross.
  • Proficiency with HomeCare HomeBase (HCHB) EMR.
  • Experience with OASIS documentation.

Work Environment

  • Dress code: Scrubs.
  • Patient visits conducted in home settings.
  • Mileage reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient not reimbursed).
  • Local candidates accepted under the same terms as travelers.

Compliance and Documentation Requirements

  • Background checks including 7-year history, sex offender, EPLS/GSA/SAM, HHS/OIG, and state-specific searches as applicable.
  • Fingerprinting required based on state regulations.
  • Education verification required.
  • Drug screening: 10-panel drug test.
  • Health requirements: Physical exam within 12 months, negative TB test or chest X-ray within 12 months, and immunization/titer documentation for Measles, Mumps, Rubella, Hepatitis B, Varicella.
  • COVID-19 vaccination or declination documentation required.
  • Seasonal flu vaccination or declination documentation required.
  • Two professional references required.
  • Work history verification for at least the last 7 years or two most recent employers.
  • Completion of various compliance documents and quizzes prior to start, including but not limited to:
    • Code of Conduct
    • Compliance Training Certification
    • HIPAA and Privacy Policy
    • Home Health Core Policy Acknowledgement and Scenarios
    • Confidentiality and Employee Responsibility Agreements
    • COVID-19 Policy and Attestation
    • TB Questionnaire
    • Other state-specific forms as applicable

Additional Notes

  • No guaranteed hours; if patient census is low, contractor may be floated to a nearby clinic to pick up hours.
  • Weekend and on-call requirements vary by location and should be discussed during the interview.
  • Must be willing to comply with all state-specific licensing and background requirements.
  • Candidates must sign new supplier packets at the start of each assignment and annually if working more than one consecutive year.

This position is ideal for experienced Registered Nurses with recent home health or hospice experience who are comfortable with travel and working independently in patient homes.
