Job Opportunity: Physical Therapist - Outpatient

Work Schedule
- Day shift, 5 days a week (9:00 AM to 6:00 PM)
- 40 hours per week guaranteed
- Must work three days with shifts ending at 6 PM; shorter days are negotiable
Job Requirements
- Valid Physical Therapist license in Michigan
- Minimum 1 year of Physical Therapist experience, including experience within the last year
- Basic Life Support (BLS) certification issued by the American Heart Association
Call, Weekend, and Float Requirements
- No call requirement
- No weekend requirement
- No float requirement
Required Documentation and Compliance
- Government-issued photo ID (Driver’s License or State ID)
- E-Verify completion as per contract requirements
- Criminal background check within 45 days of start, including lifetime search from age 18, with multiple components (SSN trace, national/statewide criminal searches, sex offender registry, education verification, motor vehicle record)
- Education verification (copy of highest degree or diploma)
- Physical exam within 1 year of start
- Drug test: 10-panel including Marijuana/THC within 30 days of start
- Immunizations and health requirements:
- Tetanus/Diphtheria/Tdap (proof of at least one prior Tdap vaccine)
- Negative TB test or chest X-ray within 30 days of start (annual TB testing not required)
- TB questionnaire updated annually
- Measles, Mumps, Rubella, Varicella: 2 vaccines or positive titer (no exemptions accepted)
- Hepatitis B: titer, series, or facility declination required
- Seasonal flu vaccination or approved medical/spiritual exemption (for candidates starting after November 1)
- Fit mask testing may be required depending on work location and role (not typically required for outpatient PT)
- References: One professional reference from a supervisor or manager who directly supervised the candidate
- Complete employment application/resume with full 7 years work history, no gaps
Certifications (as applicable)
- Basic Life Support (BLS) - AHA only (required)
- Other certifications may be required depending on unit or assignment (e.g., ACLS, PALS, NRP, TNCC, etc.) but are not specified for this role

- Candidates must complete a skills checklist annually for the applicable unit
- Candidates must review and sign the facility’s Code of Conduct and Appearance Standards Policy prior to start
- Background checks and certain state-specific clearances may be required depending on placement
- Fingerprinting via Michigan LARA required within 45 days of start if applicable (not typically required for outpatient PT)
- Some facility-specific forms and acknowledgments may be required before start
